Report Fraud, Waste, and Abuse

The City of Irwindale is committed to protecting its residents, employees, and public assets entrusted to the City. Accordingly, it is the policy of the City to identify and promptly investigate incidents of fraud, waste, and abuse committed by City employees and officials, contractors, vendors, citizens and other parties doing business with the City, and to pursue appropriate remedies available under the law. Fraud, waste, and abuse, as set forth in the City's Fraud Policy, may be reported anonymously in four different ways:

  • By calling (626) 430-2216
  • Email Fraud Hotline
  • By filling out an online form and submitting completed form online; or
  • By downloading, filling out and mailing the Investigative Complaint Form (Not yet available)

The City will not tolerate malicious reports. Reports will not be considered malicious merely because they determined to be unsubstantiated. A malicious report is one that is made in bad faith or with knowledge that the complaint entirely lacks any factual basis.

The Fraud Hotline is not a substitute for employment-related grievances or complaints by City employees, as existing City policy and procedures provide channels for resolution of these issues.

Examples of What You Can Report

  • Theft of City resources (e.g., cash, equipment, supplies or materials)
  • Record falsification
  • Payroll fraud or time abuse
  • Kickbacks or bribes
  • Intentional misuse of City equipment or property
  • Contractor fraud
  • Gross mismanagement of resources, including careless expenditures
  • Gross disregard of policy and procedural controls

Reporting Process

It is the City's intent to fully investigate every report objectively. In order to process your complaint, please provide the following information:

  • Date, Time and Place of Incident
  • Names of individuals involved
  • Description of Complaint:
    • What happened and why you think it is/was wrong
    • Who else may be aware of the incident
    • Any documentation or other evidence you have or know o
